Directions: Each parent of a Mayer Lutheran High School student is encouraged to complete the following survey. While the survey is anonymous, there is a place for you to identify yourself, if you wish to do so. If you would like us to contact you as a follow-up to any of your responses, please indicate your desire for us to do so.
