Grandparent Information Form

Mayer Lutheran values opportunities to develop and nurture relationships with our students and their families. Grandparents are no exception to this. Every spring, ML hosts a Grandparents Day, giving this generation an opportunity to join their grandchild(ren) on campus. After a special chapel and engaging program, grandparents and students enjoy a meal together. Grandparents are given a special ML gift as they leave campus with memories to be forever treasured.

This year’s Grandparents Day will be April 24, 2026. We would like to invite all grandparents of ML middle and high school students to this event. In order to send an invitation, we are seeking the addresses of grandparents of our current students.

Please complete the form below with contact information for your child’s grandparents. If your child’s grandparents are unable to be present, please include the mailing address of an older friend or relative who would like to join them for the day. There is room on the form to provide information for up to four sets of grandparents.
